This form is exclusively for nominating Service Year Option candidates (Maryland residents aged 18-24 that have earned or will earn a high school diploma/GED/certification of completion before August 3, 2026). Maryland Corps (applicants 25 and over) candidate nominations will not be considered until the Maryland Corps application cycle opens in mid-summer.

IMPORTANT: Organizations applying to host Members may not nominate anyone you are related to, nor may you nominate a current employee (full-time or part-time) of your organization.
